Safety committee meeting minutes template

Safety committee minutes should show that every reported hazard has an owner and a status, that each incident and near miss since the last meeting was reviewed for its cause, and what the committee asked management to fix. Start with the open hazard log, then cover incidents, inspection findings, employee concerns and training, and finish with action items that have one owner and a date.

Questions people ask

Should safety committee minutes name the injured employee?
No. Describe what happened, the cause and the fix. The name and medical details go on your injury records, such as the OSHA 300 log and 301 incident report if your company must keep them, and minutes are often posted on a board or emailed to everyone.
Are safety committees required by law?
It depends on where you are. Some states require them for certain employers: OSHA's 2016 review counted 14 states that require a safety committee at some or all workplaces, with rules on who sits on it, how often it meets and what records it keeps. Washington, for example, requires one where 11 or more employees work on the same shift at the same location, and in California a committee is one way to meet the injury and illness prevention program rules. Check your state's rules and ask your workers' compensation insurer; this page is not legal advice.
Why record near misses if nobody got hurt?
A near miss shows you a hazard before it injures someone. The pallet that slid off the forks with nobody in the walkway is the same hazard as the one that hits someone next month. OSHA encourages employers to investigate close calls as well as injuries, and a forklift operator involved in a near miss has to get refresher training.
What should the committee do when management does not act on a recommendation?
Keep the recommendation on the minutes with the date it went to management and the answer you are waiting for. Raise it at every meeting until there is a written yes, no or alternative. A clear record of what was asked and when is the committee's strongest tool.
